Kentucky's move to Therap and the 2026 claim denials
Kentucky's Department for Medicaid Services (DMS) now contracts with Therap for EVV, after moving off Netsmart. Agencies could start documenting visits in Therap, or sending them to the Therap aggregator, from January 1, 2025, had to finish the move by February 28, 2025, and lost access to Netsmart on April 1, 2025. Kentucky uses an open model: Therap is the state-sponsored system, and an agency's own system must meet the Cures Act and integrate with the Therap aggregator.
Enforcement for the waivers followed. From January 1, 2026, any 1915(c) waiver claim without a matching EVV visit is denied - for the Acquired Brain Injury, Home and Community Based, Michelle P. and Supports for Community Living waivers, whether services are participant-directed or traditional. Agency and financial management agency staff are responsible for making sure visits are entered.

Kentucky agencies live inside two sets of rules: Kentucky Department for Medicaid Services's, and Therap's. The software was built to both — the state's EVV requirements in the visit record, Therap's interface in the connector — so the agency does the work once and the aggregator gets it in its own format. A REST API with a token
Each visit is posted as JSON to Therap's endpoint with a bearer token; Therap answers per visit. The field names follow the state's data dictionary and are configurable on the account.
Vendor approval
Therap approves the vendor and issues the base URL, token and data dictionary under a sponsoring provider; the state then lists the system as approved.
Near-real-time
States on Therap expect visits within a day; the software queues every closed visit at clock-out and sends within minutes.
