Tennessee: from two MCO aggregators to one
TennCare delivers almost everything through its managed care organizations, so EVV in Tennessee has run through the MCOs' vendors. For home health, that meant two: BlueCare used Sandata, while Wellpoint and UnitedHealthcare used CareBridge, and an agency serving members of all three had to work with both. From August 1, 2025, TennCare, its MCOs and CareBridge moved personal care providers to a single open model on CareBridge: use CareBridge's system at no cost, or a third-party vendor that CareBridge integrates with free of charge.
Home health came in two phases. Private duty nursing and hourly nursing and aide services went live in January 2023, and all MCOs were denying non-compliant claims by November 2023. Intermittent visits went live July 1, 2023, and after extended grace periods MCOs began denying non-compliant intermittent claims on April 1, 2024. TennCare has also clarified that Medicare crossover claims are outside the EVV requirement.

Medicare crossovers are out
When Medicaid pays only Medicare cost-sharing, the EVV requirement does not apply.
Commercial primary: pay-and-chase
When commercial insurance is primary and Medicaid secondary, MCOs pay their contracted rate and recover from the primary payer, as long as the visit is recorded in EVV.
Intermittent visits are covered
Nursing, aide, therapy and social work visits (G0151-G0162, G0299, G0300, G0493-G0496, S9127-S9131, S9474) are in scope.
Integration is free
CareBridge integrates with an agency's chosen third-party vendor at no cost; the vendor's own fees are the agency's.
One aggregator: CareBridge, by file
Tennessee agencies live inside two sets of rules: TennCare's, and CareBridge's. The software was built to both — the state's EVV requirements in the visit record, CareBridge's interface in the connector — so the agency does the work once and the aggregator gets it in its own format. Open model since August 1, 2025 — one aggregator for all three MCOs.
A file, not an API
CareBridge takes a pipe-delimited visit file over SFTP and answers with a response file. The software writes the file in the state's column order, uploads it, and reads the response back into each visit.

CareBridge's field names and rules are public; the exact column order for a state is in its technical specification, handed over at onboarding. It goes on the account, not into code.

No. TennCare excludes Medicare crossover claims where Medicaid pays only cost-sharing.

Is there a free trial for Tennessee agencies?
Yes. Tennessee agencies that bill Medicaid and sign up by December 31, 2026 get 90 days free with no limit on clients: scheduling, the caregiver EVV app, documents, billing and the family portal. AI features are not part of the trial. Private-pay-only agencies get 15 days.
